A completed enrollment form and tuition agreement with a $45 non-refundable registration fee is to be on file to secure placement. Each child at Faith Early Learning Center must have the following forms completed and on file before receiving care:

Income/Eligibility form for CACFP (Child Adult Care Food Program)
